Why another 12-lead and STEMI program?

The Goal is to help individual healthcare providers and STEMI systems to:

Increase correct activation

 Decrease unnecessary activation

The Program

The program is organized into two Levels

Current Program

LEVEL I

Attention is paid to the strategies producing the biggest improvements for the least
amount of effort. The sections of this level include:

Critical thinking:
A flow chart that guides you through the ECG interpretation process used throughout the program. A model of critical thinking that integrates the patient’s clinical presentation, ECG findings, and portions of the ECG device’s statement.

STEMI Tune-Up:
It is assumed you are already familiar with the essentials of STEMI recognition. From there, we take a lesson learned approach and focus on where improvements are
often best made.

Ruling out the top STEMI impostors:
Non-ischemic causes of ST elevation are very commonly encountered. This section identifies the “usual suspects” and shows you how to rule them out in a few seconds.

The less obvious STEMI:
There is more to it than just looking for ST elevation. Understanding isolated posterior STEMI and the dynamic nature of ST elevation in STEMI increases appropriate cath lab activations.

STEMI in the presence of an impostor:
STEMI impostors can produce ST elevation when no STEMI is present. However, a STEMI may be ongoing in the presence of an impostor. This session provides initial strategies to suspect a STEMI in the presence of LVH, BBB, and other impostors.

LEVEL II

A deeper dive into the topics of Level I, along with a few new topics. These sections of this level include:

Obstructive Myocardial infarction:
In addition to ST elevation, other ECG findings can suggest the need for immediate reperfusion in the cath lab. This section looks at ECG findings for impending STEMI and STEMI equivalents. These can be collectively considered evidence for obstructive myocardial infarction (OMI).

Additional STEMI impostors:
Beyond the “usual suspects,” there are many additional STEMi impostors. This section examines stress cardiomyopathy (Takotsubo Syndrome), aortic aneurysm, pulmonary embolism, ventricular aneurysm, and more.

Suspecting STEMI in the presence of an impostor:
Picking up where we left off in Level I, we dive deeper to provide additional strategies, including: Analyzing the ST segment shape (beyond smiling and frowning), when to use concordant/discordant ST changes, Sgarbossa criteria, and others.

AV block in STEMI:
It’s not just a matter of degree! Learn to use the 12-lead to predict the location, cause, severity, stability, and appropriate treatment for an AV block caused by STEMI.
